The multidisciplinary team approach has long been the cornerstone of effective limb salvage. In their recent publication in Federal Practitioner, our friends and colleagues Neal Barshes and coworkers explore the successes and challenges of the multidisciplinary amputation prevention program at the Michael E. DeBakey Veterans Affairs Medical Center (MEDVAMC) over the past decade. Their findings offer valuable insights into how interdisciplinary care can dramatically improve patient outcomes.
Study Overview
The MEDVAMC limb salvage program began in 2011, aiming to address the high rates of leg amputations among veterans with diabetes. By integrating vascular surgery, podiatry, infectious diseases, and geriatric wound care, the program sought to reduce fragmentation in care and improve clinical outcomes.
Key Findings
1. Reduction in Amputation Rates:
• From 2012 to 2017, the incidence of leg amputations decreased by 60%, from 160 to 66 amputations per 100,000 veterans annually.
2. Enhanced Care Practices:
• The program implemented evidence-based strategies, including angiography for all patients with foot ulcers, targeted infection management, and advanced wound care techniques.
3. Demonstrated Value:
• MEDVAMC’s approach resulted in significant improvements, such as a 95% reduction in surgical wound infections and high patient trust ratings (>90%).
Challenges and Adaptations
The program faced challenges, including staff turnover and resource limitations. Despite these hurdles, the team adapted by refining protocols, embracing telehealth during the COVID-19 pandemic, and targeting high-risk populations with educational interventions.
Conclusions
Barshes et al. conclude that a sustained focus on multidisciplinary collaboration, evidence-based practices, and continuous learning can yield remarkable reductions in amputation rates. Their experience highlights the critical need for resources and systematic approaches to address disparities and improve care outcomes.
