An offloading boot can protect a foot. A well-timed nudge may help make sure it is there when the person takes the next step.
Our paper in Diabetology, “Can a Chatbot Help Heal a Wound?”, explores that space between the prescription and everyday life. Led by Aria Rabet and Aminreza Khandan with our UCLA–USC team, it asks whether messages informed by actual offloading behavior can prompt a more useful response than reminders sent on a clock. The article was published May 12, 2026, and has now appeared in our PubMed alert.
What the sensors saw
This was an eight-person feasibility case series nested within the 210-participant SmartBoot trial. A sensor-equipped offloading boot and paired smartwatch supplied behavioral data to CORA, which delivered smartphone notifications. Of 130 notifications delivered, 125 were eligible for the response analysis.
The outcome was a sensor-detected change from nonadherence to adherent behavior within 60 minutes. That could include protected weight-bearing or stopping unprotected walking to rest; it was not simply a measure of putting the boot back on.

These are responses to notifications, with repeated observations from eight people. They are not percentages of patients healed. The signal favors prompts tied to what is happening in the moment, but this nonrandomized comparison cannot separate the effects of timing, message content, and other influences.
The connection to Lifestyle Redesign
This pairs naturally with our companion post, Healing Happens Between Appointments: Lifestyle Redesign for Diabetic Foot Care, featuring Stacey Schepens Niemiec, Tze-Woei Tan, and colleagues. Their adapted occupational therapy intervention addresses how foot care fits into routines, responsibilities, and emotional well-being.
Here is the connection I find compelling: Lifestyle Redesign helps build a workable daily plan. Sensor-informed support could help reinforce that plan at the moment it is needed. Think of the therapist and patient designing the route together, with the digital system offering a timely course correction along the way.
That combination remains a research opportunity. These papers did not test an integrated Lifestyle Redesign–chatbot intervention. A useful next study would ask whether pairing the approaches improves sustained adherence, patient experience, and healing without adding an unacceptable burden.
An early signal worth following
The chatbot cohort was small and entirely male, and the study did not measure patient-reported usability or notification burden. It establishes feasibility and an encouraging short-term behavioral signal; it does not establish faster healing, fewer amputations, or durable adherence.

The next step is to test whether timely support can help turn more prescribed care into care that actually happens at home—and more hospital-free, activity-rich, dignity-filled days.
