Effect of Prior Antibiotic Use on Culture Results in People with Diabetes and Foot Osteomyelitis #ActAgainstAmputation @LimbsandLungs @BradSpellberg

Great work from our long time colleague and the pride of Amsterdam, Prof. EJG Peters and coworkers.

Here is a summary:

  • Background: Antibiotic use prior to biopsy in diabetic foot osteomyelitis (DFO) patients may affect culture results and bacterial resistance
  • Methods: Prospective analysis of ulcer bed and percutaneous bone biopsies in DFO patients, investigating the impact of prior antibiotics on culture results and resistance
  • Sample: 64 people with DFO, 29 of whom received prior antibiotics
  • Results:
    • Prior antibiotics did not increase the risk of at least one negative culture (RR 1.3, CI 0.8-2.0)
    • No increased risk for specific types of negative culture:
      • Bone cultures (RR 1.15, CI 0.75-1.7)
      • Ulcer bed cultures (RR 0.92, CI 0.33-2.6)
      • Both cultures (RR 1.3, CI 0.35-4.7)
    • No increased risk of antibiotic resistance in combined bacterial results (RR 0.64, CI 0.23-1.8)
  • Conclusions: Antibiotics given up to 7 days before biopsies in DFO patients do not impact culture yield or antibiotic resistance, regardless of biopsy type
