A Closer Look at Diabetic Foot Ulcer Treatment Adherence: Insights from Singapore
Diabetic foot ulcers (DFUs) are a severe complication of diabetes, affecting millions globally and leading to significant morbidity and healthcare costs. Effective management of DFUs is complex and requires adherence to a multidisciplinary treatment plan. However, patient adherence to DFU treatment is a challenge, and understanding the factors influencing adherence is crucial for improving outcomes.
A recent systematic review led by Gwyneth WY Ng and colleagues, including senior author Zhiwen Joseph Lo from Singapore, provides valuable insights into this issue. The study, published in The International Journal of Lower Extremity Wounds, aimed to identify and classify factors affecting patient adherence to DFU treatment according to the World Health Organization (WHO) Dimensions of Adherence to Long-Term Therapies[1].
Methodology and Findings
The review analyzed 643 articles from PubMed, Embase, and Scopus, focusing on both qualitative and quantitative studies that discussed factors affecting patient adherence to DFU treatment. The factors were organized into five WHO adherence domains: patient-related, condition-related, therapy-related, socioeconomic, and health system-related.
The review found that the most studied factors were patient-related, including patient insight, motivation, and perception of DFU treatment. Therapy-related factors, such as the use of offloading footwear and reminder devices, also played a significant role in adherence. Socioeconomic factors, including social support and income, and health system-related factors, such as the patient-provider relationship and treatment availability, were less studied but recognized as important.
Implications for Clinical Practice
The study’s findings underscore the importance of a comprehensive approach to DFU management that considers the various factors influencing adherence. Clinicians and healthcare providers can use this information to tailor interventions and support systems to improve adherence and, ultimately, patient outcomes.
