We are pleased to announce the publication of a new manuscript titled “The Who and the How: Understanding Populations and Implementing Limb-Preservation Programs to Maximize Success” by our colleagues Leigh Ann O’Banion, MD, and Christian Campat, DO, from Fresno, CA. This invited commentary, published in the Journal of Vascular Surgery, explores the critical elements required for successful limb-preservation programs.

Key Insights from the Manuscript:

  • Limb Preservation as a Vital Principle: The authors emphasize that limb preservation is a key principle of vascular surgery, highlighting the high 5-year mortality following major amputation and the significant socioeconomic burden of limb loss.
  • Understanding Target Populations: A deep understanding of targeted patient populations is crucial for the success of limb-preservation programs. The study highlights the steep rise in both minor and major amputations in Oklahoma from 2008 to 2019, identifying key risk factors such as uninsured status and Native American ethnicity.
  • Socioeconomic and Social Support Factors: The manuscript points out that disadvantaged, uninsured populations and those without strong social support networks are at higher risk of amputation. Engaging patient stakeholders and incorporating social workers and care coordinators into multidisciplinary networks are recommended to address these challenges.
  • Comprehensive Care Networks: The authors advocate for developing multidisciplinary networks that identify patients early, engage in shared decision-making, and provide comprehensive care to meet patient needs. These networks should be equipped to handle both limb preservation and functional outcomes for patients undergoing major amputation.

Future Directions:

The commentary stresses the importance of ongoing efforts to improve patient care for those with chronic limb-threatening ischemia, not only in Oklahoma but throughout the United States. By targeting the most at-need patient populations and engaging stakeholders, we can build more effective limb-preservation programs.
