The Validity and Reliability of Self-Reported Adherence to Using Offloading Treatment in People with Diabetes-Related Foot Ulcers

Summary:

  • Objective: Assess validity and reliability of self-reported adherence to removable cast walker (RCW) offloading treatment in people with diabetic foot ulcers (DFUs)
  • Participants: 53 people with DFUs using RCWs
  • Methods: Comparing self-reported adherence with objectively measured adherence using dual activity monitors; test-retest reliability assessed in a subset of 19 participants after one week
  • Results:
    • Median self-reported adherence greater than objectively measured adherence (90% vs. 35%, p < 0.01)
    • Fair agreement for validity (r = 0.46, p < 0.01); large 95% limits of agreement with significant proportional bias (β = 0.46, p < 0.01)
    • Minimal agreement for test-retest reliability (K = 0.36, p < 0.01)
  • Conclusion: People with DFUs significantly overestimate their adherence; clinicians and researchers should use objective adherence measures instead
